PG&E is reporting a widespread outage affecting thousands of customers in Berkeley and many City buildings, including the Finance Customer Service Center and the Permit Service Center. Call respective City services for further details, or check the PG&E outage page. Power is expected to return by 12:45pm. Traffic lights that are not working should be treated as a four-way stop sign.

 How to Request a Birth or Death Certificate

The City of Berkeley Health Officer has ordered residents to shelter in place, leaving only for essential needs.  Vital Records is an essential need. Most City facilities are closed to the public. Read the latest changes to City services and COVID-19 information from Berkeley Public Health.

The Office of Vital Statistics will provide birth and death certificates by appointment only on Mondays, Tuesdays and Thursdays. Appointments will be scheduled after a phone screening. The office is closed every Friday and on all City Holidays.

Birth certificates and death certificates can also be requested by mail or by internet.  Click on the links below for detailed instructions.

IN PERSON - By appointment only; VALID PICTURE ID IS REQUIRED

Instructions for scheduled appointments:

1. If you have an application, please complete section 1 and 2 before arriving; if you do not have one we will provide one at the appointment or you can also download the application form.

2. Bring in payment of $27.00 for each birth certificate, $23 for each death certificate requested at the time of making your appointment in the form of a personal check or money order made out to: City of Berkeley
     ◦ We are NOT accepting cash or credit, debit cards at this time.
     ◦ Amount of certificates requested cannot be changed during the appointment

3. Bring in valid picture identification.

4. Please wear a face covering.

5. Appointment address: 2180 Milvia St. Berkeley, CA 94704.
    ◦ Please be on time as we have one appointment every 15 minutes.
    ◦ The front doors are locked.  Please wait outside of the front doors or if you are let in please wait in the lobby. One of our staff will come out and get you at your appointment time
  


BY MAIL

Send the application form with a notarized sworn statement (*) with your check or money order made payable to City of Berkeley.  Indicate the number of copies you want.  Your request will be processed within 4-6 weeks from receipt.  Please include your current and correct return address.  We cannot be responsible for returned mail. You have up to three (3) months to follow-up on your request.

(*) The Notarized Sworn Statement is only required if you are requesting Certified Copies of Birth or Death Records.  There is no need of notarization if you are requesting Still Birth or Fetal Death Record.

NOTE:  Out of country payments must be made in U.S. dollars.

All orders received will be processed for shipping within 7-10 business days.  Express delivery fee is $27.00 (subject to change by overnight carrier).   International and Saturday deliveries may be arranged for an additional fee.  The Express delivery carrier will not deliver to a P.O. Box address.
