HIPAA Breach at the High School Health Center

A binder in the Berkeley High School Health Center that tracks some medications given to students cannot be found. The binder went missing between February 20th and February 21st 2019. This binder contains the full name of 190 patients, the medications prescribed, and the dates of the prescriptions.

The binder contains no other personal information at all.

The Berkeley High School Health Center has reached out to impacted patients but does not have sufficient or current contact information for all students. If you were a patient between July 2018 and February 2019, please call toll free (833) 889-8235 to determine if your information may have been affected. Patients may also visit the Berkeley High School Health Center during normal business hours.  

There is no evidence any unauthorized person has looked at this binder. Nonetheless, we want to let patients know this incident occurred. We are sorry this happened, and we have taken a number of steps to ensure that this type of error cannot happen again.

The Berkeley High School Health Center has provided free and confidential services to students for decades, and we are committed to offering excellent care.  A primary goal is to be trusted by students.

With this in mind, this information breach concerns us greatly. We took immediate action to put in place training and procedures to prevent a similar incident from occurring in the future.
