Declaration of Paternity - Adding Father's Name

A Declaration of Paternity form is used to legally estabish the paternity (the father) of a child when the mother and the father are not married to each other.   It should be signed by the biological mother only if she is not married.  It may be signed by the biological father regardless of his marital status.  Signing the Declaration of Paternity is a voluntary act, and by doing so you are will be giving up some rights that you may have, including the right to a court trial to decide the issue of paternity.   For a detailed and complete information about your rights, please refer to the Declaration of Paternity form or consult an attorney.

Do I need to make an appointment?

No.  You may just walk in and request the forms, however many people have difficulties on filling those forms out, and they end up making mistakes and having to do it all over.   For that reason, we highly recommend that you come in at least 30 minutes prior to closing time and request one of our staff to review your documents for accuracy.  It's a simple process for us, and it's free of charge as well.

Office hours for Declaration of Paternity: 

9:30AM to 3:30PM


  1. Plan accordinly and allow yourself at least 30min prior to closing time, so we can review your documents.  
  2. No appointments are needed.  
  3. First come first serve basis.
  4. There is a fee of $23 that needs to be sent along with the documents to the CA Department of Public Health.  Our office do not charge anything for reviewing and witness you signing the documents.
