FREE HEALTH SCREENINGS AND RESOURCES AT SATURDAY EVENT IN SOUTH BERKELEY
The Heart 2 Heart Program Hosts a Community Celebration with Food, Fun, and Resources

Berkeley, California (Thursday, April 14, 2016) - The Heart 2 Heart Program is hosting a neighborhood celebration on Saturday, April 23, 2016 where people can receive information, resources, and health screenings, such as screenings for blood pressure, blood sugar, cholesterol, and Hepatitis C.

Those who come can also connect to resources and services, including those from the City of Berkeley and LifeLong Medical Care, or one of the 20 local health and social service agencies on site. People will also get assistance with applications for Covered California, the state health insurance exchange created by the Affordable Care Act that provides federally subsidized rates.

Heart to Heart Health Fair
11 am - 3 pm
near the entrance to the Frances Albrier Community Center
2800 Park Street in South Berkeley.

Enjoy free food, courtesy of Acme Bread, and delicious smoothies created by our "Smoothie Bike." Children can take part in our "Kid Zone" which will be filled with fun activities, games, and face painting. Dance to the music that DJ Freddy will spin throughout the event, and participate in our free raffle - with prizes donated by prominent regional businesses! Free haircuts will also be available, courtesy of stylists from "DnD Cuts" - a local barbershop near South Berkeley!

This celebration represents the sixth anniversary of the Heart to Heart Mobile Health Van, which hosts events on the corner of Oregon Street and Sacramento in Berkeley, typically on the 4th Saturday of the month from 11:00 AM - 1:00 PM.

Heart 2 Heart (H2H) is a unique collaboration between the City of Berkeley Public Health, LifeLong Medical Care, and the Heart 2 Heart community in South Berkeley. Working together, we bring a holistic, community-based approach to addressing inequities in hypertension and heart disease focused on South Berkeley.  H2H connects residents with needed programs, resources, and services.  For more information on Berkeley Health Outcomes, please see the Health Status Report published by City of Berkeley Public Health.

Other H2H projects include our monthly Barbershop Health Hubs, which occur every 3rd Friday of the month from 11 - 1 pm, at the three barbershops and 1 hair salon in South Berkeley. Similar resources and services are offered at the monthly Mobile Health Van, as well. We also hold an annual job and education fair in the fall each year - with the intent to connect participants to employment, educational, and vocational opportunities. We have a mini-grant program, in which we support community driven efforts that are in-line with reducing health inequities in the H2H neighborhood. Lastly is our health advocate program, in which we train people that live, work, play, or pray in the Heart 2 Heart community on various health related topics (i.e. chronic disease prevention, hypertension, navigating through the health care system, etc.) with the intent of them imparting this information to others within their community - with the overall goal of supporting and empowering a healthy community.
